Florida Blue has expanded its cancer support program for Medicare Advantage members in partnership with Evolent, a company focused on supporting individuals with complex health conditions. The program provides members with a dedicated care navigator and access to digital tools at no extra cost, aiming to offer ongoing support between doctor visits. Early data indicates improved patient outcomes, reduced healthcare costs, and a 95% satisfaction rate among patients and caregivers.
The initiative addresses challenges faced by older adults diagnosed with cancer. As cancer rates rise with age, many seniors also experience social isolation. According to the U.S. Census Bureau’s 2022 report, over a quarter of Americans aged 65 or older live alone without children, making it harder for them to manage their care independently. In Florida specifically, more than 1.1 million older adults were living alone as of 2023, according to the Florida Department of Health.
Since beginning their collaboration in 2021, Florida Blue and Evolent have aimed to improve cancer care for members. The new addition of care navigation offers personalized assistance such as help scheduling appointments, managing symptoms and side effects, preparing for emergencies, and connecting patients to local resources like support groups.

The program also incorporates Careology, a digital platform that allows patients to track symptoms, set reminders for medications or appointments, share updates with their care team in real time, and receive advice outside regular office visits. This system aims to ensure timely support even when patients are not at the clinic by allowing navigators to identify issues early.
“By giving clinicians real-time insights into a patient’s experience, we can help them make better decisions and provide more effective care,” said Dr. Von Nguyen, Evolent’s chief medical officer. “This improves patient outcomes and enhances the overall care experience for patients and their families.”
